Unusual properties of accessory pathways.
نویسندگان
چکیده
accessory fibers may be quite variable. The unusual responses to electrical stimulation in these patients included: 1) failure to depolarize the His bundle from the site of ventricular pre-excitation; 2) supraventricular tachycardia with two types of re-entry; 3) anterograde block in the accessory pathway with intact retrograde conduction; and 4) retrograde delay or block in the accessory pathway and atrioventricular (A-V) node with intact anterograde conduction. Understanding these variations may provide important therapeutic implications regarding drug and surgical management. In addition to these anomalies in conduction over accessory pathways we demonstrated in these patients the occurrence of retrograde functional right bundle branch block and retrograde "gap' phenomenon, and ventricular reciprocation without recording intervening atrial discharge.

Radiofrequency catheter ablation in the treatment of Wolff-Parkinson-White syndrome
Introduction: Tachyarrhythmias in Wolf Parkinson White (WPW) syndrome, can be a life threatening factor. Antiarrhythmic drug therapy in this syndrome, has not been completely acceptable. Efficacy, safety and economy of Radio Frequency Catheter Ablation (RFCA) in western studies, has made it as the treatment of choice. In the present study, efficacy of RFCA in the ablation of accessory pathway...
متن کاملAccessory atrioventricular pathways that conduct only in the antegrade direction.
Patients with accessory atrioventricular pathways that conduct only in the antegrade direction represent an unusual variant of the Wolff-Parkinson-White syndrome. This report describes such patients and compares them with patients with accessory pathways that demonstrate bidirectional conduction. Of 143 patients with single accessory pathways, seven demonstrated exclusive antegrade conduction (...
متن کاملLooks Like VT But Isn't - Successful Ablation Of A Left Free Wall Accessory Pathway With Mahaim-like Properties
It was long believed that Mahaim pathways represented nodo-fascicular or nodo-ventricular connections. However, this misconception was challenged when patients underwent surgical or catheter ablation of the AV node but remained pre-excited. Electrophysiology (EP) studies showed these pathways to be right sided decrementally conducting atrio-fascicular accessory pathways with the atrium forming ...
متن کاملDifferential electrophysiologic properties of decremental retrograde pathways in long RP' tachycardia.
Long RP' supraventricular tachycardias (SVT) often demonstrate both slow and decremental conduction properties in the retrograde pathway of the reentrant circuit. The electrophysiologic properties of these pathways are poorly understood. We studied 10 patients with long RP' SVT (RP'/RR, 0.52 to 0.71); five had the unusual form of atrioventricular nodal reentry (fast-slow) and five patients had ...
متن کاملIdentification of Pathway Fiber Orientation
The ability to record accessory atrioventricular (AV) pathway activation consistently may be uniquely beneficial in improving pathway localization, identifying anatomic relations, and providing insight into unusual conduction properties. For the purpose of recording left AV accessory pathway activation, an electrode catheter was specially designed for use in the coronary sinus. The orthogonal c...
